ABSTRACT

The aim of this paper is to carry out a sociological analysis of the implications of what could be referred to as the paradox of farm labourers during the pandemic. That is, their designation as essential workers turned them simultaneously into high-risk workers and a source of public alarm. Based on the analysis of the logics of purity and danger of Mary Douglas (1991), this article analyses how some of the public health strategies developed by the institutions during the pandemic contributed to reinforcing the invisibility and vulnerability of migrant workers. The article shows how, being placed in the realm of the impure and dangerous, foreign agricultural workers were not able to receive the health care they required, and their living spaces thus became environments conducive to the spread of the virus. Through an exploration of secondary sources, a review of pertinent documents and in-depth interviews with key informants, this article firstly offers a description of the institutional process by which foreign agricultural workers in the Region of Murcia were converted into essential workers facilitated the spread of infections. Secondly, it explores the link between the precariousness of the working and living conditions of farm labourers and the absence of the ethics of self-care, which was at the heart of the chain of Covid-19 infections suffered by this population. Finally, it focuses on institutional responses to the threat posed by the threat posed by increased infections among immigrant farm workers. Copyright ©2022 CSIC.

ABSTRACT

The study includes the analysis of cultural, commercial, and safety factors that affect the potential of the trade of the red sour pitaya (Stenocereus gummosus), as well as its characterization, consumer patterns, consumption channels, and the relevance, identity, and cultural value of the fruit in the Municipality of La Paz, Baja California Sur, Mexico. The investigation has the objective of understanding the perception of the product in local knowledge, which is identified in the study area, where the wild fruit takes an added value in informal commercialization and is amalgamated with the local culture. For the analysis and data collection, a simple random sampling was conducted, on 227 economically active people with purchasing power. The method of applying the instrument was digital, limiting face-to-face contact with people due to the current Covid 19 pandemic. The exercise characterizes a population with mostly university studies, which consumes the product in a traditional way, which reflected a greater appreciation of the pitaya fruit, by older people.

ABSTRACT

Copyright © 2022

ABSTRACT

Background: Since the beginning of the SARS-CoV-2 pandemic, identifying the COVID-19 pathophysiology not only has been addressed to applying diagnostic tests or preventing through vaccines, but also to the timely detection, especially of patients in risk groups such as those in transplants areas (renal, hematology, etcetera). In the case of these patients, using RT-PCR tests avoids putting them at risk by subjecting them to states of immunosuppression that could aggravate their situation if they were faced with an onset of a COVID-19 infection.

ABSTRACT

Introduction Current healthcare settings and ICUs especially are complex, highly technical, and multidisciplinary, with interactions between healthcare professionals and users, in which there may be errors at different levels. Our objective was to assess the perception of patient safety in our unit at the end of the third wave of the COVID pandemic, with the intention of conducting subsequent improvement actions. Methods Observational, cross-sectional, and descriptive study. The perception of Safety Culture was estimated using the HSOPS questionnaire translated into Spanish. Some questions were posed in a positive sense, and others in a negative sense. The response was also rated as positive, negative, or neutral. Our findings were compared visually, not mathematically, with those found in the previous national study «Analysis of the culture on patient safety in the hospital setting of the Spanish National Health System» published in 2009. A subgroup analysis was performed according to professional group and seniority as a health worker. The Student's t, χ? and ANOVA tests were used. Results Sixty-two professionals responded to the questionnaire, 73.90% of the total. The median time working in ICU 2 years (interquartile range 2-4.5 years). The rating for the degree of safety was 8.06 (SD 1.16). The majority (91.20%) had not reported any adverse event in the last year. A total of 30.90% had recently received patient safety training. The dimensions considered as weaknesses were 9 («Staffing», with 27.57% of positive responses) and 10 (“Support of the hospital management in safety”, with 17.64% of positive responses). The dimensions considered as strengths were 3 («Expectation of actions by management / supervision of the service») with 85.29% of positive responses, and 5 («Teamwork») with 95.58% of positive responses. The Cronbach's alpha index values suggest that the questionnaire has adequate internal consistency. In general, our data are more positive than those collected in the 2011 national survey, although the 2 dimensions considered weaknesses were already considered such in the previous work. Conclusions The perception of patient safety in the ICU of our hospital after the end of the third wave of the COVID pandemic is adequate, with a more positive rating than that of the national study on safety culture at the hospital level carried out in 2009. The constant quest for patient safety should prioritize activity in the 2 dimensions considered weaknesses: staffing, and support from hospital management in everything related to patient safety.

ABSTRACT

Background and aims: The pandemic caused by SARS-COV2 is responsible for a considerable impact on stroke care. During this time many other non-COVID diseases have been neglected. Primary health care centers collapsed in some regions and the control and monitoring of vascular risk factors has been difficult. On the other hand, it is also known that covid infection leads to a certain prothrombotic state that could result in subsequent strokes. Our objective is to evaluate the effect of the SARS-CoV-2 pandemic on stroke unit admission rates. Methods: Temporal admission rates in the stroke unit of a regional hospital were analyzed from 2019, 2020 and 2021. The type of stroke, baseline characteristics and treatments received were recorded. Results: Stroke unit admission rates were similar between 2019 and 2020: 397 and 408 patients respectively. However, in 2021 the number of stroke admission, Ischemic or hemorrhagic, has incremented by more than 30% (p-value 0,005), with 589 attended patients. Hemorrhagic stroke rates were 15.62%, 10.70% and 16.98% in the three years respectively. Conclusions: During the first year of SARS-CoV-2 pandemic the use of health services for neurological emergencies was reduced, included the number of admissions at the stroke unit. However, we are experiencing a significant increase in the number of strokes, which now far exceeds prepandemic levels. It remains to be determined whether the cause of this huge increase in stroke cases is related to suboptimal vascular risks factor control, to COVID-related factors or to optimal population response following stroke campaigns in the region.

ABSTRACT

Involving students in collaborative projects and experiential learning helps develop transversal skills that, with passive learning, do not usually have specific training or reinforcement. Additionally, certain transversal skills such as the ability to communicate and coordinate in interdisciplinary work groups are essential for our graduates in their professional activity. The project consisted of a simulation of a real work environment in which a multidisciplinary team, made up of students from two different subjects of different degrees, must coordinate their efforts to propose a coherent solution to a problem. This project aims to reinforce the skills and competencies necessary both for the development of the rest of the subjects and for the subsequent exercise of the profession. These skills require reinforcement not only from the teacher but also from the classmates themselves, who would be competitors, students and the jury at the same time when assessing the results of the rest of the students. The problem to be solved focused on a proposal related to the supply of energy for a hypothetical agricultural industry, where agricultural engineering students carried out a design project for an electrical installation and the building students participated in the project by conducting an energy audit. Through collaborative work, it has been seen that students show a total willingness to work with students of other degrees and that the learning process improves since there is a greater interaction of skills and knowledge, which gives them an interdisciplinary vision of what it would perform a real job. Despite the difficult situation motivated by COVID pandemic, the project was successful and in total 25 students participated demonstrating that cooperative learning improves transversal skills.

ABSTRACT

Background and importanceThe COVID-19 pandemic has had a significant impact on cancer diagnosis and treatment worldwide.Aim and objectivesTo describe patients in oncology treatments comparing 2019, 2020, and 2021 to September 2021.Material and methodsA descriptive study was conducted in a tertiary hospital from January 2019 to September 2021. Inclusion criteria were patients undergoing parenteral and oral oncology treatment. Variables were: gender, age, diagnosis, patients with oral and parenteral oncology treatments dispensed by the Pharmacy Service. Data were collected from the electronic medical record (FarmaTools).ResultsDuring the study period, 1010 patients were treated with parenteral and 402 with oral antineoplastics. The average age was 67±23.7 years (51.6% male). In the group of parenteral treatments the main diagnoses in 2019 were: vesical carcinoma (VC) (14%), metastasic non-small cell lung cancer (mNSCLC) non-squamous (NS) noALK noEGFR (7.1%) and metastatic KRAS and NRAS mutated colorectal cancer (mCC) (6%);in 2020 were VC (13%), mNSCLC NS noALK noEGFR (6.2%) and adjuvance in breast cancer (mBC) noHER2 and positive hormonal receptor (+HR) (5.6%);and in 2021 (to September) VC (9%), metastasic NSCLC NS noALK noEGFR (5,9%) and KRAS and NRAS mutated mCC (5.9%). In treatment with oral antineoplasics in 2019: adjuvance CC (27.6%), mBC noHER2 +HR (20%) and metastasic castration-resistant prostate cancer (mCRPC) (10.5%);in 2020 mBC noHER2 +HR (20%), adjuvance CC (12.5%) and mCRPC (10.5%);and in 2021 mBC noHER2 +HR (21%), adjuvance CC (11.8%) and mCRPC (7.8%). 364 patients were treated by an intravenous route in 2021, 356 in 2020 and 290 in 2021. 105 patients were treated by oral treatment in 2019, 144 in 2020 and 153 in 2021. Patients treated in metastasic stages were 241 in 2019, 254 in 2020 and 234 in 2021.Conclusion and relevanceIn 2020, there was a decrease in patients treated with KRAS and NRAS mutated mCC and an increase in adjuvance BC. Regarding oral treatment, patients on adjuvant treatment with colorectal cancer decreased in 2020. The increase in the number of patients on oral treatments from 2019 to 2021 is notable, and the important role that telemedicine has had from 2020 and the home delivery of medication by pharmacy services, thus reducing hospital visits. Further studies are needed to confirm this.References and/or acknowledgementsConflict of interestNo conflict of interest

ABSTRACT

Background and Study Aim The rising of physical inactivity among university students amidst Covid-19 pandemic is a worldwide concern. Thus, the purpose of this research is to describe and explain the status quo of university students in terms of their physical activity engagement level. Material and Methods Descriptive research design was used in the study employing survey questionnaire. There were 1,648 respondents of the study from one of the Higher Educational Institutions in Cebu City, Philippines taking physical education subject. Results Based on the survey conducted, results revealed that in spite of the presence of pandemic, university students still engage themselves in different exercise typologies and/or physical activity in varying frequency of execution to stay active. Similarly, household chores are considered as an opportunity in physical activity engagement amidst the pandemic as revealed in the study. Conclusions Despite the challenges and online modality of learning, university students engaged in different exercise typologies to stay active in varying frequency means. Understanding university students' physical activity's inclination amidst the pandemic entails planning new teaching strategies and approaches which cater their interest to encourage and motivate to live an active lifestyle even staying at home with limited access to facilities. Further, the study recommended an exercise program to be used either as teaching tool for effective teaching strategies and approaches for students' physical activity interest in online learning modality especially in this global pandemic.

ABSTRACT

Background: The KEYNOTE 426 trial demonstrated pem 200mg plus axi twice daily (bd) is effective in aRCC. Pharmacokinetic modelling of pem 400 mg every six weeks demonstrated similar observed exposures to pem 200 mg every three weeks. In NHS Scotland, pem6w + axi was used during the COVID-19 pandemic to reduce hospital visits and for pts' convenience. Here, we evaluate the RWE of pem6w + axi in aRCC. Methods: Electronic medical records of pts on pem6w + axi between 1 May 2020 & 1 Jun 2021 in two large cancer centers in Scotland were analyzed retrospectively for pts' characteristics, treatment related adverse events (TRAE) and efficacy. Results: Total of 93 pts were identified (Table). At data cut off of 8 Sep 2021, with a median follow-up of 7 months (mo) (interquartile range (IQR) 5 mo;range 0-15 mo), 73 pts (78%) were alive. Clinician assessed radiological response was evaluable in 87 pts. Overall response rate (ORR) including partial response + complete response was noted in 49 pts (56%) with median time to response of 80 days (d). Clinical benefit rate including pts with ORR and stable disease was seen in 72 pts (83%). At data cut off, 24 pts (26%) had progressed with a median time to progression of 117 d (IQR: 85d). Median overall and progression free survival were not reached and follow-up continues. 81 pts (87%) had any grade and 28 pts (30%) had grade 3/4 TRAE. Immune related AE (irAE) of any grade occurred in 60 pts (65%) and grade 3/4 in 19 pts (20%). Common grade 3/4 irAE were transaminitis (10%), colitis (8%), nephritis (2%) and skin (2%). 28 (30%) and 14 pts (15%) respectively required steroids and hospitalisation for irAE with median hospital stay 6 d (range: 2-43 d). 10 pts suffered a second irAE requiring steroids and 5 pts had a second hospital stay with median hospital stay of 6 d. Any grade AE and grade 3/4 AE related to axi occurred in 67 (72%) and 14 pts (15%) respectively. Axi dose was escalated from 5mg to 7mg bd in 12 pts (13%), reduced to 3mg bd in 35 pts (38%) and 2mg bd in 10 pts (11%). 21pts (23%) on Pem and 15pts (16%) on axi discontinued treatment due to TRAE. Conclusions: Our RWE demonstrates that pem6w + axi appears to have comparable safety profile to pem 200mg + axi reported in Keynote 426 study, with the added benefits of less frequent hospital visits. Further follow up continues for efficacy in this heterogeneous pts population. Pt characteristics, total=93 (%).

ABSTRACT

COVID-19 pneumonia is associated with severe respiratory failure that requires orotracheal intubation and assisted mechanical ventilation. Among the risks of prolonged mechanical ventilation, one of the most feared and challenging is the development of a tracheoesophageal fistula due to the respiratory and nutritional alterations that it entails. The diagnostic method of choice is bronchoscopy. Spontaneous closure of the tracheoesophageal fistula is rare and surgical management is generally required for its definitive correction. We present the case of a patient with COVID-19 pneumonia who developed a tracheoesophageal fistula and required surgical management as definitive treatment. © 2021, Instituto Nacional de Enfermedades Respiratorias.
